Committee Nominations 2010 Arrow Icon


The SAM AGM is on 16th February 2010.

Nominations for new Committee members should be completed and sent/give to the SAM Secretary:-


1. Click on the PDF icon below, to download and print a copy of the nomination form.

2. Complete the form.

3. Make sure it is signed both by proposer & seconder.

4. Hand or post the form to the SAM Secretary in time for the AGM.

NB Please make sure you only propose someone with their knowledge and agreement.
